This sounds very difficult but it is actually very easy.

You need to just practice a small exercise a few times a day to train the gag reflex mechanism to function properly when the end of the mouth is being touched. The exercise is to 1st to lift the back of your tongue and touch it in the upper part of your mouth.

Then you had to breathe through the nose but you had to keep your mouth open. You had to be in this position for a longer period of time and ideally for as long as you can. While your mouth is open the tongue should be in the same position that is the back of the tongue should be touching the upper part of the mouth.

You had to practice this exercise multiple times a day so that you can train your gag reflex mechanism to react properly in case it is triggered by the touching of the end of the mouth.

You cannot directly does the toothbrush at the far end of the mouth because that would cause gagging and therefore you had to just start first with the area just after the teeth and then day by day you have to touch the toothbrush more and more further so that you are able to train the gag reflex mechanism to work properly without gagging.
